Coming Home completed pilot program with Hendricks Regional Health

Coming Home completed the pilot program with Hendricks Regional Health in March 2009. The Coming Home Medical Organizer was given to patients at the time of discharge. The organizer allowed them to utilize the provided forms (and accompanied CD templates) necessary to maintain all of their medical information. The program was a huge success!

Our friends at Doozie, Marketing & Design were instrumental in the creation of the Coming Home Medical Organizer.

Coming Home in Partnership with Hendricks Regional Health

Coming Home partnered with Hendricks Regional Health to begin a pilot program using our new Coming Home Medical Organizer. The Coming Home Medical Organizer will be given to the patient at discharge time and allow them to utilize the provided forms (and accompanied CD templates) necessary to maintain all of their medical information.
For people with chronic conditions and unique illnesses, information contained within the Medical Organizer is invaluable. This information can go with them to any emergency or hospital visit, doctor visits, family events and trips; so that they are never without critical healthcare information.
